Job Description
Greenkey Resources LLC is seeking a Palliative Care Nurse Practitioner or Physician Assistant in Belleville, NJ. The role involves leading a palliative care team, providing mentorship, conducting patient consultations, and collaborating with interdisciplinary teams. Candidates should have a Master's in Nursing, active NP or PA license in New Jersey, and at least 5 years of palliative care experience. The position is full-time, primarily Monday to Friday.
Overview
Serve as a clinical leader for an advanced practice provider team in palliative care, focusing on patient-centered, evidence-based interdisciplinary medicine.
Provide mentorship and guidance to Nurse Practitioners and Physician Assistants, ensuring quality standards and consistent documentation practices.
Engage in direct patient care, including consultations and follow-ups, calibrated to team size and operational needs.
Collaborate with interdisciplinary teams to deliver holistic care across hospital settings, including ICU and ED encounters.
Lead advance care planning conversations, coordinating with external care providers when necessary.
Shape clinical standards, mentor staff, and contribute to program development within the palliative care department.
Participate in academic pathways, CME opportunities, and leadership growth initiatives.
Work primarily Monday–Friday, with flexibility to adjust workdays as needed.
Key Responsibilities & Duties
Provide leadership and clinical support to advanced practice providers in the palliative care team.
Carry a focused clinical caseload, delivering patient-centered care and evidence-based practices.
Conduct palliative care consultations and follow-up visits across hospital units.
Lead goals-of-care discussions and advance care planning with patients and families.
Ensure quality standards and consistent documentation practices across the team.
Support onboarding, mentorship, and case review processes for team members.
Build and maintain referral relationships with hospital partners and external providers.
Promote evidence-based standards and interdisciplinary collaboration within the department.
Job Requirements
Master of Science in Nursing with active New Jersey NP or PA license and national board certification.
Minimum of 5 years of palliative care experience, with leadership or mentoring experience preferred.
Palliative care certification (e.g., ACHPN / CHPN) is highly desirable.
Strong communication, clinical judgment, and interdisciplinary collaboration skills.
DEA and CDS registration or ability to obtain prior to start date.
Experience in program-building and quality oversight within a healthcare setting.
Commitment to delivering holistic, patient-centered care in a hospital environment.
Flexibility to adapt to a dynamic and growing team structure.
