Job Description
The Community Health Educator at Sustainable Resources Center in Minneapolis, MN, focuses on combating lead poisoning through in-home visits, case management, and community education. Responsibilities include conducting assessments, managing client information, engaging in community events, and teaching classes. Candidates should have strong communication skills, a valid driver's license, and experience in outreach or case management is preferred. This full-time role requires personal vehicle use for client visits.
ORGANIZATION SUMMARY:
Sustainable Resources Center (SRC) is a Minnesota non-profit that has been administering energy and healthy homes programs for 48 years and has helped tens of thousands of families live in safer and more efficient homes.
Lead poisoning in children continues to be a national health crisis. The Community Health Educator plays a critical role in the fight against this crisis. SRC’s Education & Outreach program includes community-based lead poisoning screening and education, in-home visits, case management and referral for lead hazard control services. SRC’s clients are primarily low-income and receive all services free of charge.
PRIMARY JOB DUTIES:
In-home Visits: Schedule and visit client homes to identify lead paint hazards, conduct healthy homes assessments, and deliver education & light duty healthy homes products and materials.
Case Management: Manage the collection of client information and facilitate enrollment in grant application process to have lead hazards removed from the home and/or healthy homes improvements made to the home at no cost to the resident. Persistent contact and follow-up with clients is required.
Community Events: Actively engage with the public and event attendees by providing information and answering questions about lead poisoning prevention and inviting kids and pregnant individuals for free blood lead testing performed at SRC’s van via finger stick by a qualified medical professional. SRC has the only mobile lead testing van in the nation – the “Leadie Eddie” van.
Community Education: Assist with and eventually lead the teaching of Lead Safe Work Practices classes, Healthy Homes classes, and other educational presentations.
QUALIFICATIONS:
- Strong outgoing and extroverted personality, comfortable making follow-up phone calls as well as engaging with event attendees and leading classroom training.
- Ability to relate to and communicate effectively with individuals from many different cultures and socioeconomic situations.
- Valid driver's license, relatively clean driving record and daily access to an insured automobile.
- Experience with Microsoft Word and Excel.
Preferred Qualifications:
- Prior experience in community outreach/education and/or case management
- Recent graduate (undergrad or grad school) with strong education in Community Health, social work or related field along with solid academic achievement.
- Bi- or multilingual fluency, particularly Spanish, Hmong or Somali.
ADDITIONAL INFORMATION:
This is a full-time position eligible for paid time off, paid vacation, paid holidays, 401(k) with employer match, employer-subsidized medical and dental insurance, employer-paid life insurance, and short-term and long-term disability benefits. Starting wage is $20.00 - $21.00 per hour depending on qualifications.
- Some evening and weekend hours are required to facilitate community events.
- Community Health Educators drive their own vehicles throughout the community to visit clients - a personal vehicle is required for this job. Mileage is reimbursed at federal rates. Educators work with and report to SRC’s Director of Lead Education & Outreach Department.
