Your Role & Responsibilities

As a Home Health Physical Therapist, you’ll play a crucial role in improving patients' mobility and quality of life by delivering in-home therapy that promotes independence.
• Personalized Treatment Plans: Design and implement customized therapy plans to help patients meet their mobility goals.
• Therapeutic Exercises: Guide patients through exercises and recovery techniques to enhance strength and movement.
• Patient and Family Education: Teach patients and families how to maintain mobility and wellness long-term.
• Collaboration: Work closely with physicians, nurses, and the care team to ensure coordinated, high-quality care.
