ADS Health Aide - Per Diem

The Health Aide position at La Posada is a per diem role within the Adult Day Services program, designed for individuals who are passionate about providing care to seniors. This position requires a dependable individual with a keen attention to detail, effective prioritization skills, and a strong ability to work as part of a team. The Health Aide will operate in a well-equipped and aesthetically pleasing environment, ensuring the delivery of high-quality care to clients in the Adult Day Health Care Program. In this role, the Health Aide will work under the direct supervision of the Adult Day Services Administrator and will assist clients with daily living activities as outlined in their treatment plans. These activities may include ambulation, mobility, medication administration, personal hygiene, and social interactions. The Health Aide will also be responsible for communicating any changes in the participants' conditions to the program administrator and interacting with the participants' families according to established guidelines. The position emphasizes the importance of effective communication, teamwork, and a genuine affinity for working with seniors. The Health Aide will also participate in case reviews to evaluate clients' needs and plan for ongoing services, ensuring that the care provided aligns with the mission and goals of the program.

Responsibilities
• Assist with personal care, such as toileting, oral care, applying lotion, ambulation, or personal hygiene assistance.

• Prepare and maintain records of client progress and services performed, reporting changes in client condition to the administrator, supervisor, nurse, or caregiver.

• Communicate effectively and positively with participants, their family members, staff, and volunteers to enhance the well-being of the participants.

• Perform light housekeeping duties as needed to keep the facility clean and hazard-free, which may include cleaning, sweeping, or mopping.

• Perform healthcare-related tasks, such as monitoring vital signs and weights, under the direction of a nurse.

• Take clients on trips outside the building, assisting with transportation as needed.
,
• Instruct or advise clients on issues such as cleanliness, hygiene, nutrition, or care.

• Participate in case reviews, consulting with the team caring for the client to evaluate the client's needs and plan for continuing services.

• Provide clients with communication assistance as needed.
