Job Description

CORE (Community Organized Relief Effort) is a nonprofit organization whose mission is to build sustained, community-based response programs to natural disasters. In response to the COVID-19 pandemic, CORE GA provides free vaccinations and additional resources to disadvantaged communities and emerging hotspots around Georgia. This is accomplished through maintaining strategic partnerships with local health agencies and community-based organizations to reach high-risk populations in vulnerable areas.The Community Health Worker will provide chronic health screenings and other CLIA waived tests participants, as well as distribute and provide education around the utilization and importance of Narcan. Community health workers typically have deep roots or shared life experiences in the communities they serve. This role will work within the mobile unit to decrease barriers to health equity in their communities. ### Responsibilities:

  • Provide chronic health screenings and other CLIA waived tests toparticipants.
  • Lead in the distribution of blood pressure, blood glucose testing equipment and Narcan.
  • Organize and facilitate health education workshops around opiate issuesusing culturally appropriate terms and concepts.
  • Provide community members with information and knowledge on a range of health-related subjects.
  • Link participants to Health Outreach Coordinators for resource services specific to their unique needs via Intake Forms and direct referrals.
  • Establish and maintain positive relationships with community members, local organizations, and healthcare providers.
  • Engage with partners formally and informally.
  • Participate in CORE team goals and objectives.
  • Adhere to site team schedule.
  • Follow all CORE procedures regarding operations and logistics.
  • Report to the Team Supervisorfor direction and communicating significant incidents.
  • Participate in daily site operations and adapt to changes.
  • Submit data reports and maintain proper storage of paperwork
  • Assist the team with set up, execution and breakdown.
  • Support site team members with site development, scheduled, patient registrations, incentive distribution, in addition to other duties related to operations.
  • Other duties and special projects as assigned.

Scope and Travel

  • Comfortable traveling up to 1-3 hours from work base in personal vehicle

Qualifications:

  • Prefer certification of CNA, MA or LPN
  • Available Full-Time, 40 hours a week, including weekends (weekly schedule varies, 5 days/week)
  • Live in, or near the local health district office
  • Fluency in another language, particularly Spanish is preferred
  • Access to reliable transportation to travel throughout assigned Health Districts
  • Comfortable working outdoorsthroughout the year
  • Knowledge of medical office management systems and procedures
  • Lift up to 30bs

Experience:

  • A minimum of 1 year experience as a Medical Assistant, Certified Nursing Assistant, Licensed Practical Nurse or Community Health Worker
  • Experience in educating community members about health topics

Skills and Competencies:

  • Excellent analytical skills, observation and analysis capacity
  • Very good knowledge of G-Suite and Microsoft Office (including MS Word, Excel)
  • Readiness to commit and adhere to the values, mission and vision of CORE
  • Excellent writing, reporting and communication skills
  • Excellent time management skills and ability to multi-task and prioritize work
  • Ability to exhibit a professional, positive attitude and work ethic.
  • Ability to initiate and have culturally appropriate conversations related to the topic at hand.
  • Excellent interpersonal skills required and ability to interact professionally with culturally diverse individuals and individuals in distress.
  • Stay adaptable to changes in policy, procedure, and protocol.
  • Ability to engage 1:1, in groups, during events, etc.
  • Ability to communicate openly and maintain good relationships within the team and partners.
  • Responsive, motivated, willingness to take responsibility.
  • Ability to cope with stress, adhere to deadlines and respect CORE management.
